I give permission for my child to attend Pleasant Valley Church TeamKID. I attest that my child(ren) is/are physically capable to participate in this event and will notify PVC leadership if there is a change in their physical health that would prevent them from participation in any aspect of TeamKID. I give permission for my child’s details to be stored by PVC (this may be on computer) as considered necessary for contact purposes. Furthermore, in the unlikely event of illness or accident I give permission for any necessary emergency first aid or medical treatment to be given. In an emergency and if I am not contactable, I am willing for my child to receive hospital treatment. I understand that every reasonable effort will be made to contact me as soon as possible concerning my child(ren). I further acknowledge that the kids ministry at PVC  is led by volunteers, rather than by paid, trained professionals. In consideration for accepting the registration of the named individual(s) and permitting the voluntary participation of said individuals, I (for myself as well as for my child, his heirs and assigns) hereby release, discharge, and hold harmless Pleasant Valley Church and their employees and volunteers, from and against any and all claims arising out of or relating to illness, physical injury, death or other damages that may arise while participating in TeamKID. I affirm that my child will adhere to PVC’s expectations of appropriate conduct. I affirm that my child is capable of both following the guidance of the ministries leadership and will respect the rules of the church to maintain a holistic, safe and nurturing environment for all.
